Information for State-Approved Mosquito Control Programs

Florida mosquito control programs seeking to maintain state-approved status are required by statute and administrative code to submit specific documentation to the Florida Department of Agriculture and Consumer Services (FDACS).

Mosquito control programs must submit the required documents prior to the submission deadline in order to obtain or maintain state-approved status, with or without state aid. According to sections 5E-13.022 and 5E-13.027, Florida Administrative Code, failure to adhere to the deadlines may result in denial of state aid and/or state approval.

If interested in becoming a state-approved program, fill out the Preliminary Information Sheet for State Approval [ pdf 187.3 kB ]  and follow the submission guidelines listed in the document. The information you provide on the form will assist in the approval process.
