Questionnaire for Appointment to an Advisory Council or Committee
Completion of the form is required for all prospective appointees to advisory boards, councils, authorities or committees with decision-making responsibilities.
The first page of the form is to be used to comply with reporting minority representation on boards, commissions and committees pursuant to Section 760.80, Florida Statutes.
The second page is information we need to determine your qualifications for the desired appointment and basic background information.
The form must be completed in full. Answer "none" or "not applicable" where appropriate. Please complete both pages of the form.
