Application for Prior Permission to Bring Swine into Florida
Attention
If traveling with swine that are not consigned directly to a U.S. Department of Agriculture (USDA) recognized slaughter facility, you must have a health certificate to continue this application for entry permit.
This form is to be filled out to request movement of swine into the state of Florida. All addresses must be verifiable 911 addresses for the permit to be valid.
